A. Mandatory Documents (For All Candidates)
All candidates must keep the following documents ready before filling out the NEET UG 2026 application form:

1. Passport-size Photograph

• Recent photograph
• White background
• Preferably taken within the last 6 months

2. Postcard-size Photograph

• Same as the passport-size photograph
• As per NTA specifications

3. Scanned Signature

• Signed using blue or black ink
• On plain white paper

4. Left and Right Hand Finger Impressions

• Clear and properly scanned

5. Class 10 Mark Sheet

• Used as proof of date of birth

6. Class 10 Passing Certificate

7. CGPA to Percentage Conversion Certificate (If applicable)

• Required if marks are provided in CGPA format

8. Valid Photo Identity Proof (Any one of the following):

• Aadhaar Card
• Passport
• PAN Card
• Voter ID
• DigiLocker ID

9. Present and Permanent Address Proof

B. Additional Documents (If Applicable)
The following documents are required only for candidates claiming
reservation or special status:

10. Category Certificate

• SC / ST / OBC-NCL / EWS
• Must be issued in the Central Government prescribed format

11. PwBD Certificate

• For Persons with Benchmark Disabilities
• Issued by an authorized medical authority

12. Domicile Certificate

• Required for State Quota counselling

13. Citizenship Certificate

• For NRI / OCI / Foreign National candidates

C. Special Requirements for NEET UG 2026

14. Live Photograph Capture During Registration

• Candidate must capture a live photograph using a webcam or mobile device

15. Aadhaar-based e-KYC

• Aadhaar authentication is mandatory

• Incorrect or blurred uploads may lead to rejection of the application.
• Candidates are advised to keep both soft copies and hard copies
ready for reference.
